Please cancel the direct debit immediately and reverse the debit to my/our account processed after the direct debit was cancelled. Any interest and/or fees charged to my account as a result of the incorrect processing of the direct debit must also be reversed.
I await your confirmation in writing within 7 days of the date of this letter that:
The direct debit to [name of merchant] has been cancelled; and
Any amounts debited to my/our account (under this direct debit) after it was cancelled are credited to my/our account and any interest and/or fees charged on the debit also refunded to my/our account.
