A copy of this letter is attached for your information [attach copy of letter]. I also attach a copy of my letter to [name of merchant] dated / / for your information. In that letter I cancelled a direct debit from my credit card account to [name of merchant]. A further direct debit to [name of merchant] has been processed to my/our account since I notified you and [name of merchant] that the direct debit was cancelled. Please chargeback the debit to my account processed after the direct debit was cancelled. As you can see the debit was unauthorised. Any interest and/or fees charged to my account as a result of the incorrect processing of the direct debit must also be reversed. I await your confirmation in writing within 7 days of the date of this letter that the amounts debited to my (under this direct debit) after it was cancelled are credited to my account and any interest and/or fees charged on the debit also refunded to my account.
