RE: CANCELLATION OF DIRECT DEBIT

I refer to my letter dated / / [date of letter cancelling direct debit]. A copy of this letter is attached for your information [attach copy of letter].

In that letter I cancelled a direct debit from my/our account to [name of merchant].

A further direct debit to [name of merchant] has been processed to my/our account since I notified you that the direct debit was cancelled.

Please cancel the direct debit immediately and reverse the debit to my/our account processed after the direct debit was cancelled. Any interest and/or fees charged to my account as a result of the incorrect processing of the direct debit must also be reversed.

I await your confirmation in writing within 7 days of the date of this letter that:

The direct debit to [name of merchant] has been cancelled; and
Any amounts debited to my/our account (under this direct debit) after it was cancelled are credited to my/our account and any interest and/or fees charged on the debit also refunded to my/our account.
